For students who do NOT attend Canisius High School:

Important Notes:

  • Payment for courses can be made via credit card after this form is submitted.
  • Tuition will not be refunded for applications withdrawn after June 3, 2020.
  • A student is not considered registered until tuition is paid in full.
  • Canisius High School reserves the right to cancel a class for insufficient enrollment.
  • Writing the Common Application Essay Workshop will be held August 10 – 14 from 9am-12pm.

Students may NOT be absent more than twice during the course schedule. Additional absences will result in credit being denied. Please note: The HEALTH course requires attendance on June 29 and July 29; absence on either date precludes a student from receiving course credit; refunds will not be issued. One absence is allowed but discouraged for the Health Course.
